Introduction

Transfer of blastocysts may be a promising new technique for certain patients in which it is difficult to make the selection of the best embryo purely based on the morphology on day two or day three after IVF or ICSI. At this moment the embryos are still largely dependent on the maternal genome, and the full transition to the embryonic genome has only taken place after the 8 cell stage. Therefore when genetic damage exists on the embryonic genome this sometimes may only become evident at or after the morula stage.

As we have shown that the morphology of day 3 embryos for blastocyst formation is limited some patients may benefit from postponing the transfer provided the culture conditions are optimal to sustain embryonic growth at that stage.

However it will be self-evident that follow-up of pregnancies obtained after transfer of blastocysts is imperative. We therefore call upon all centers that perform transfer of blastocysts to participate in the registry
